You must submit the following:
Lifetime License Additional Requirements
If you are applying for a Lifetime License, you must also submit the following:

Copy of current anti-rabies certificate (including the date of the vaccination, expiration date, and name of clinic where the vaccination was done)

Copy of microchip implantation paperwork (including brand name, chip number, date of implant, and name of clinic where the implant was done)
If separate veterinarians were used for the above services, statements from all will be necessary.
Service Dog Lifetime License Additional Requirements
If you are applying for a Service Dog Lifetime License, you must also submit a copy of the Certificate of Training. A microchip is not required, but the information will be recorded if the implant paperwork is submitted.
