Application Requirements
You must submit the following:
- Fully completed Lifetime Dog License Application
- Written veterinarian proof that the dog has been spayed or neutered (Required)
- Dangerous Animal Annual Registration Fee
- Fully completed and notarized Dangerous Dog Notification / Agreement Form
- Proof of liability insurance in the minimum amount of $300,000 - Certificate of Insurance
- Copy of microchip implantation paperwork (including brand name, chip number, date of implant, and name of clinic where the implant was done) Dog must be microchipped.
- Copy of current anti-rabies certificate (including the date of the vaccination, expiration date, and name of clinic where the vaccination was done)
- Signed and notarized Ramsey County Dangerous Animal Registration Form
- Required Lifetime License Fee
