If you are not renewing the lifetime license tag for the dangerous or potentially dangerous dog listed on the renewal form, you are required to submit a written statement which includes the reason for not renewing.
The notification must include:
- Current owner's name and address
- Relocation address
- Name of the new owner, if any
- Date and cause of death, if the dog has died
