Application Requirements
You must submit the following:
- Fully completed Dog License Application
- Written veterinarian proof if the dog has been spayed or neutered
- Copy of microchip implantation paperwork (including brand name, chip number, date of implant, and name of clinic where the implant was done) Dog must be microchipped.
- Copy of current anti-rabies certificate (including the date of the vaccination, expiration date, and name of clinic where the vaccination was done)
- The required Lifetime License fee
