Information to Record

You may wish to write down the following information to keep on file:

  • Date of complaint call
  • Address of problem
  • Name of owner (if you know)
  • Name(s) of tenant, owner, or other person involved in the problem (if you know)
  • Type(s) of problem (be as specific as possible)
  • How long this has been a problem
  • When it occurs (day of week, time of day, specific dates)
  • City department or office called and with whom you spoke (if you know)
