Grievance Form: American with Disabilities Act (ADA)

If you are unable to access a City program, service, or activity due to a disability, please let the ADA Title II Coordinator know via the form below, by email adacoordinator@ci.stpaul.mn.

Examples of issues that should be reported:

Physical

  • a lack of wheelchair ramps or damaged ramps
  • inaccessible entrances

Program & service issues

  • Being denied City benefits or services because of a disability
  • Having an accommodation request denied to participate in a City program

Communication

  • An American Sign Language (ASL) interpreter not being provided at a public meeting upon request
  • Forms and information not being provided in an alternative format upon request
