Overview
A building or portion of a building designed and equipped for the conduct of sports, exercise, leisure-time activities, or other customary and usual recreational activities, operated as profit or non-profit, and which can be open only to bona fide members and guests of the organization or open to the public for a fee.
An adult health / sports club (Class B) is a health / sports club which excludes minors by reason of age. If you have questions, contact DSI at 651-266-8989, or email the DSI Message Center.

Application Requirements
- Information needed for your online application:
- Proof of current Secretary of State registration and Assumed Name Filing
- Class N License District Council Form
- Business Purchase Agreement (Only for Change of Ownership applications)
- Property Lease / rental agreement or proof of ownership
- Licensing General Liability Insurance Certificate
- Environmental Health Management Plan
- Written proof that all employees are at least 18 years of age, if applying for a Class B (Adult) License

Inspections
Your application will be administratively reviewed by DSI Licensing, Zoning and Fire Inspection staff. This review may include on site inspections(s) to verify compliance with applicable regulations. You will be informed of any required inspections including the name and telephone number of the inspector after the submission of a completed license application. The location must be in compliance with all applicable regulations and/or license conditions before any license may be issued.

Expiration/Renewal
A Health / Sports Club License will expire one year from the date of issuance and may be renewed annually.
